Senior Case Manager

Location: London & Home Counties (Hybrid)

Salary: Up to £58,000 per annum + quarterly performance bonus of up to 10%

Hours: Full Time – 35 hours per week

The Role

An exciting opportunity has arisen for an experienced Senior Case Manager to join a specialist case management provider supporting individuals with complex disabilities.

This is a hybrid role combining home working, attendance at the company's Oxfordshire head office and travel throughout London, the Home Counties, Essex, Kent, the Midlands and the South to support clients and meet the needs of the service.

You will manage your own caseload whilst providing guidance and support to the wider Case Management team, helping to deliver high-quality, person-centred rehabilitation services.

Key Responsibilities

Manage a caseload of clients with complex disabilities.
Complete assessments and develop rehabilitation goals in partnership with clients and their families.
Produce and maintain comprehensive Case Management Plans with costed recommendations.
Commission, coordinate and monitor rehabilitation services.
Maintain accurate, compliant and up-to-date client documentation.
Build effective relationships with clients, families, commissioners, referrers and multidisciplinary teams.
Develop referral opportunities through networking and relationship building.
Provide guidance and supervision to Case Managers.
Participate in client reviews and annual appraisals alongside the Clinical Lead.
Promote clinical governance, quality improvement and safe practice.
Provide cover for the Clinical Lead during periods of absence.

About You

A minimum of 2 years' experience within the Case Management sector.
At least 5 years' experience within a relevant professional field.
Proven experience providing guidance and supervision to a team.
Excellent communication, organisational and relationship-building skills.
Strong clinical reasoning and the ability to manage a varied workload.
Knowledge of the litigation process.
Knowledge of safe recruitment practices.
An understanding of the Case Management sector and stakeholder expectations.
