Job Description - Service Coordinator

Position: Service Co-ordinator

Reports to: Head of Supported Living

Department/service: Operational Team

Purpose:

To coordinate all Support teams to ensure that the service is safe at all times and the people we support have the appropriate staffing.

Duties and responsibilities:

Rostering

1. Ensure the service is staffed appropriately so that continuity of support is maintained.

2. Ensure that assigned staff have the rights skills, qualifications, compliance and training to support the people we support.

3. Liaise with the senior management to ensure that there is sufficient cover to deliver all visits and that new people we support are planned into the rota in a timely manner. Advise on recruitment needs as required.

4. Complete and distribute rotas monthly to all appropriate parties. Approach appropriately skilled staff to request for them to cover shifts. Communicate any changes to rotas to the senior management team.

5. Ensure staff can attend shifts as rota’d and sign in accordingly

Training and Development

6. Enrol all new starters to complete mandatory training and follow-up as necessary in a timely manner to ensure full completion. Arrange with the Senior management for sign off on key competencies with the relevant people.

7. Progress new starter inductions including arranging shadowing shifts. Ensure final sign off of induction and competency is completed by an appropriately trained staff member.

8. Conduct regular review of staff training records, enrol those requiring refreshers within expiry deadlines.

9. Coordinate training for support teams including face to face events, virtual events etc.

10. Provide regular updates as required on training data, completion rates, etc.

Supervisions

11. Quality monitoring of people we support

12. Together with the senior management, conduct supervisions and appraisals of the support team and ensure lessons learned and action plans are recorded and acted upon accordingly.

13. Together with the senior leadership team, participate in monthly team meetings with the support teams to provide and gather feedback, people we support updates, and ensure lessons learned and action plans are recorded and acted upon.

14. Maintain regular communication with the support teams to offer support, knowledge or to coordinate the same as required.

15. Complete regular observations on the staff team to ensure that the support offered is to a high quality.

Administration

16. Carry out spot checks on people we support files to ensure they are complete and updated regularly.

17. Ensuring that our services complies with CQC requirements: ensure staff receive proper induction and mandatory training, and that appraisals and supervisions are completed in line with requirements

18. Make sure that the online system is regularly updated and that the information inputted is accurate, including record of testing

19. Ensure staff have access to accident/incident reporting and are aware of the procedure. Report any accidents at work to the senior leadership team.

20. Ensure that all incidents are acted upon and that debriefs are completed as well as updating the supporting documentation.

21. Liaising with professionals involved in the people we support to ensure that they are kept up to date with relevant information.

22. Ensure the compliance of all documentation in line with current guidance set out by CQC and other frameworks.

Recruitment

23. Identify appropriately skilled/trained and compliant staff to meet Care Planning requirements. Review current staff to establish their suitability and availability and ensure any vacancies are recruited to.

24. Participate in the shortlisting and interviewing process, identifying potential candidates.

25. Conduct safer recruitment checks of applicants and contribute to interview questions, ensuring any queries are addresses.

Additional

26. Participate in the on-call rota. Provide out of hours support to staff and people we support as necessary. Escalate enquiries to the senior leadership team where additional support is needed.

27. Support on the ground, supporting the team, covering shifts where cover is needed.

28. Support with other tasks as reasonably requested by the Senior leadership team

29. Visiting all services on a regular basis to ensure that the people we support have the opportunity to voice any concerns that they may have
