Your new company We're seeking a committed and skilled Social Worker to join the Adult Community Team in Oxfordshire. This is an excellent opportunity to make a real impact within the community-supporting adults to live independently, safely, and with the right networks around them. Working within Adult Social Care, you'll use a strength‑based approach to carry out assessments, coordinate support, and collaborate closely with health partners, families, carers, and community organisations. You'll play a vital role in promoting wellbeing, resilience, and positive outcomes for the people you work with.
Your new role As part of the Community Team, you will:Complete strength‑based assessments, reviews, and interventions.Support individuals under the Care Act 2014, MCA 2005, and relevant legislation.Develop clear, outcome‑focused support plans tailored to individual needs.Work with multidisciplinary teams, community partners, and families to achieve the best outcomes.Provide advice on Assistive Technology, equipment, reablement services, and housing adaptations.Manage a varied caseload, including duty responsibilities and community visits.Maintain accurate, defensible case records and contribute to service development.
What you'll need to succeed You're a confident, resilient social worker who is passionate about delivering high‑quality social work and empowering individuals.Essential:Recognised Social Work qualification & Social Work England registrationStrong knowledge of Adult Social Care legislation and safeguardingAbility to work both independently and within a multidisciplinary teamExcellent communication, assessment, and report‑writing skillsCommitment to professional development and reflective practice
