Social Worker

The Assistant Social Worker (ASW) will support vulnerable adults across the borough through the implementation of our strengths\-based model of practice. This will involve:

• Conducting assessments, reviews, and developing support plans that help residents and their carers maximise independence, build on their strengths, and improve quality of life.
  




• Assisting qualified social workers with complex cases, providing hands\-on support and contributing to effective case management.
  




• Working closely with partner organisations to develop strong relationships and support innovative approaches to meeting residents’ needs and improving outcomes.
  

Key Responsibilities

• Undertake initial and ongoing assessments of adults in need of social care support.
  




• Develop, review, and update person\-centred support plans, ensuring they reflect the resident’s strengths, preferences, and goals.
  




• Identify risks and safeguarding concerns and contribute to safeguarding processes.
  

• Support social work teams in safeguarding adults at risk, including participation in strategy discussions, risk assessments, and multi\-agency meetings.
  




• Monitor and report on safeguarding concerns, ensuring timely interventions in line with statutory guidance.
  

• Build and maintain effective relationships with residents, carers, and multi\-agency partners.
  




• Support creative approaches to service delivery and contribute ideas to improve outcomes for residents.
  




• Attend relevant meetings, forums, and training to stay informed of best practice and statutory requirements.
  

• Maintain accurate records, case notes, and reports in accordance with organisational policies and statutory requirements.
  




• Participate in supervision, reflective practice, and continuous professional development.
  




• Work within the professional standards and codes of conduct for social work practice.
  

Requirements

• Social Work qualification (CQSW, DipSW, or equivalent) with HCPC registration.
  




• Minimum 2 years post\-qualification experience in adult social care, assessment, and safeguarding.
  




• Experience working with vulnerable adults and knowledge of safeguarding legislation and practice.
  




• Ability to assess needs, manage risk, and contribute to support planning.
  

• Strong interpersonal and communication skills, able to build rapport with residents and partners.
  




• Ability to work independently and as part of a multi\-disciplinary team.
  




• Problem\-solving and creative thinking to support positive outcomes.
  




• Good organisational and time management skills, with attention to detail.
  

• Experience in a local authority or health/social care setting.
  




• Knowledge of the Care Act 2014 and other relevant statutory frameworks.
  




• Familiarity with strengths\-based and person\-centred approaches.
