Job Description - Social Worker - Adoption Team

1-2 days in the office | 6 Month Rolling Contract | Adoption Team | Supportive Management | Flexibility Encouraged 

Leaders in Care are working with a forward-thinking Local Authority in North Wales s that is seeking an experienced Social Worker to join their Adoption Team on an interim basis to support with Post Adoption Support

Key Responsibilities

Undertake assessments of prospective adopters, including PAR (Prospective Adopter Reports).
Support the matching process and contribute to family finding for children with a plan of adoption.
Work closely with regional adoption agencies and other professionals to ensure timely and stable placements.
Provide post-adoption support to children, families, and adoptive parents.
Maintain accurate, timely, and high-quality case records in accordance with statutory requirements.
Attend relevant panels and contribute to care planning processes.Requirements

Qualified Social Worker with current Social Work England registration.
Demonstrable experience in adoption, family finding, or permanence planning.
Strong knowledge of relevant legislation and statutory guidance (e.g., Adoption and Children Act 2002).
Excellent written and verbal communication skills.
Ability to work autonomously and as part of a multi-agency team.Offer & Benefits

Competitive hourly rate of £38.43 P/H.
Hybrid working model – just 2 to 3 days per week in the office.
