Job Title: Social Worker - Adult Community Team
Location: Wakefield
Rate: £35 per hour
The Wakefield Council is seeking a qualified social worker with experience of working with adults to join their team.
What will your responsibilities be?
In this role, you will engage in strengths-based conversations with service users, families, and advocates to determine eligibility and support planning. You will undertake Capacity and Best Interest Assessments in line with the Mental Capacity Act 2005, conduct NHS Continuing Healthcare checklists, and participate in Decision Support Tool (DST) meetings. Additionally, you will manage a caseload effectively within a fast-paced environment and collaborate with a multi-disciplinary team, including health, pharmacy, housing, Age UK, and Carers Wakefield.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience. You must have relevant experience working with adults. You should have a thorough understanding of the Care Act 2014 and the Mental Capacity Act 2005, along with experience in undertaking Capacity and Best Interest Assessments. Familiarity with NHS Continuing Healthcare, including checklists and DST meetings, is essential. You must have a proven ability to manage a caseload effectively within a fast-paced environment, and possess strong communication, organizational, and interpersonal skills.
