Social Worker - Home First Hospital Team - Scunthorpe

We have an exciting opportunity for a dedicated Social Worker to join the Home First Hospital Team in North Lincolnshire.

Responsibilities:
You will complete information gathering on the wards. You will hold MDT (multidisciplinary team) conversations with professionals to decide the pathway for a safe and timely discharge. Additionally, you will be able to respond to A&E and plan for care from the hospital.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
