Location: Nottingham
Contract: Full-time, 37 hours per week
Working Pattern: 3 days face-to-face
Are you an experienced Social Worker looking for a rewarding role within a supportive and fast-paced environment? We are seeking one skilled practitioner to join a dynamic team in Nottingham, focusing on hospital-based work and Discharge to Assess (D2A).

🔍 About the Role

You will play a key part in supporting safe and timely hospital discharges, carrying out high-quality assessments, and working collaboratively across health and social care systems.

✅ Key Requirements

Qualified Social Worker (registered with Social Work England)
Strong experience in assessment work
Solid knowledge of Discharge to Assess (D2A) pathways
Experience working within hospital settings
Proven ability in multi-agency working
Good understanding of Continuing Healthcare (CHC) processes
Full UK driving licence, access to a vehicle, and willingness to travel both within and outside the Nottingham area
