Job Title: Hospital Social Worker
Location: Warwickshire
Rate: Up to £34 per hour
Our client in Warwickshire is seeking an experienced Hospital Social Worker to join their team.
What will your responsibilities be?
As a Hospital Social Worker, you will conduct thorough social care assessments to determine the needs of patients preparing for discharge. You will develop and implement tailored care plans and collaborate with medical, nursing, therapy, and discharge teams to ensure a smooth and timely transition from hospital to home or care settings. Managing a caseload effectively, you will also undertake additional assessments such as Mental Capacity Assessments, Safeguarding enquiries, and Continuing Health Care checklists. This role operates within a hybrid working model, requiring flexibility between home-based and hospital site work.

Qualifications and Experience:
To be successful in this role, you must hold a recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW. You must be registered with Social Work England and have significant experience in delivering social work services to vulnerable adults, including those with complex needs. A strong understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005, is essential. You should possess excellent assessment and analysis skills and be able to work effectively within a multi-disciplinary team. A full UK driving licence and access to a car are often required for client visits.
