Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disability team, based in Leeds.
What will your responsibilities be?
Completed specialist assessments for adults with learning disabilities, identifying care and support needs, promoting independence, and developing person-centred care plans in line with the Care Act 2014.
Managed a diverse caseload, undertaking reviews, safeguarding enquiries, Mental Capacity Act assessments, and risk assessments to ensure the safety and wellbeing of service users.
Worked in partnership with healthcare professionals, families, providers, and community agencies to coordinate effective support packages, facilitate transitions, and improve outcomes for individuals with learning disabilities.
