Responsibilities:

• Conducting assessments to identify vulnerabilities, risks, and needs of clients

• Developing, implementing, and reviewing care plans designed to safeguard clients from harm

• Coordinating with other service providers, agencies, and professionals to achieve the best possible outcomes for clients
