Job Title: Support Planner - Community Led Support Team 2
Salary: £34,638 - £39,309 per annum
Grade: D
Contract Type: Permanent  
Working Hours: 37 hours per week 


About the Role


Thurrock Adult Social Care are seeking an exceptional individual to join us as a Support Planner in our Community Led Support (CLS) Team 2. This role is an interesting opportunity to work in a team that is committed to connecting individuals with their communities.


The team is about responsive social care support that creates opportunity for vulnerable people to come together with their community to explore opportunities and outcomes, while providing support that is committed to safeguarding the most vulnerable.   


As part of a CLS team you will be working in a way that is responsive and proportionate, working with people to achieve their outcomes using their strengths and their community creativity, linking people with their communities focusing on ‘what matters rather than what’s the matter’. 


What We Are Looking For


You will need to be an excellent communicator and able to work alongside colleagues in other departments, health, community partners and agencies in order to meet the care and support needs, and outcomes for our residents. 


You will need to have an understanding of the social model of disability and the concept of person-directed care or the delivery outcomes to adults. Experience of working in a Health & Social Care setting is also essential for this role. 


Essential Qualifications:


A Social Care qualification NVQ level 3 or recognised Social Care or Health qualification is essential for this role.
