The role supports individuals in accordance with personalized plans, emphasizing independence and collaboration with health and social care professionals.
Responsibilities:
Deliver care and support aligned with personalized support plans.
Perform personal care, domestic tasks, and facilitate independent living skills.
Work in an enabling manner to maximize independence.
Collaborate closely with health and social care professionals involved in care planning.
Report directly to the Team Leader or Assistant Manager.
Exemplify best practices in strengths\-based and person\-centered approaches.
Connect individuals with informal, community\-based support.
Engage in partnership with residents, carers, community organizations, and professionals.
Participate in occasional weekend or evening events on a voluntary basis.
