The Role

As a Specialist Support Worker, you will provide tailored support to individuals in line with their Person-Centred Support Plans, Risk Assessments, and personal goals, helping them maintain independence, dignity, and choice.

Key Responsibilities:

* Read, understand, and follow individualised Person-Centred Support Plans and Risk Assessments.

* Support service users to express their feelings, choices, ideas, and opinions using their preferred communication methods.

* Encourage independence and support individuals with daily living activities, including:

* Preparing meals

* Domestic tasks

* Shopping

* Accessing the local community and health services

* Exploring interests and hobbies

* Supporting social interactions

* Encourage and enable individuals to make day-to-day choices and important life decisions.

* Provide personal care support, including washing, dressing, and toileting, while maintaining dignity and respect.

* Support medication through monitoring, prompting, and recording in line with policies and procedures.

* Complete household duties including cleaning, laundry, and maintaining a safe environment.

* Support individuals who may display behaviours that challenge, using positive and person-centred approaches.

* Complete accurate and factual records, including incident reports.
