Job Description - Housing Health & Wellbeing Apprentice

Housing, Health & Wellbeing Apprentice

Location: Severn House Mandale Park Belmont County Durham DH1 1TH

Salary:£10.85 per hour

Contact- Fixed term contract for duration of apprenticeship-16/06/2028

Hours: 37 hours per week Monday to Friday

Please note: The salary listed reflects the full earning potential for this role. Starting salaries depend on experience and progression within the band.

About the role

To contribute effectively as part of a team while developing the skills and knowledge required to deliver holistic support and case management to individuals with identified housing, health, and wellbeing needs. The role involves engaging directly with members of the community to provide guidance, information, and access to appropriate services, supporting them to improve their overall outcomes.

Duties include;

  • To study towards and achieve the Community Health and Wellbeing Worker Level 3 Apprenticeship Standard and complete required training to fulfil all other key accountabilities set out below.
  • Work closely with the full range of workers and skills within the service teams, including external agencies, family members and significant others as appropriate.
  • Assist Support Coordinators to carrying out comprehensive assessments with service users.
  • Work towards leading effective case management for allocated service users, including on-going processes such as risk assessments, risk management plan, support planning and safeguarding. This includes the effective use of an online case management system.
  • Providing support and helping service users access housing, healthcare, benefits, and wellbeing services.
  • Building trust with the people you support
  • Supporting the delivery of housing, health and wellbeing focused group sessions.
  • Encouraging positive health and wellbeing and independence.
  • Working with a wide range of services including housing officers, support workers, and healthcare teams.
  • Undertake continuing professional development including participating in supervision, performance reviews and attending training as/when required.

What you'll need to succeed in the role;

  • Functional skills (Maths & English) Level 2 (essential)
  • Ability to use Microsoft Office programmes(essential)
  • Good communication & empathy(essential)
  • An interest in supporting others in relation to housing, health and wellbeing.
