Key Responsibilities:

• Conduct thorough assessments of patients' physical, emotional, and social needs

• Develop and implement personalized treatment plans to enhance recovery and independence

• Collaborate with a multidisciplinary team to deliver patient-centered care

• Educate and support patients, families, and carers on treatment and recovery

• Maintain accurate patient records and contribute to discharge planning processes

Requirements:

• HCPC registration as an Occupational Therapist

• Excellent communication, organization, and interpersonal skills

• Ability to manage a varied caseload and collaborate effectively within a team

• Dedication to continuous learning and professional development
