We are seeking a skilled Band 6 Occupational Therapist to join the Care of the Elderly (COTE) team. This role focuses on discharge planning and rehabilitation, working within the orthopaedic OT team to support patients in acute medical and elderly care settings.

• Facilitate effective discharge planning for elderly patients.
• Provide OT interventions within the orthopaedic and rehab setting.
• Conduct assessments and develop care plans to promote patient independence.
• Work collaboratively with multidisciplinary teams to ensure seamless patient care.

• HCPC-registered Occupational Therapist.
• Experience in acute medical, elderly care, rehabilitation, or rapid assessment.
• Strong discharge planning and general OT skills.
• Ability to work effectively in a multidisciplinary team.
