We are looking for an experienced Band 6 Occupational Therapist to join the Home Services team at Cwm Taf Morgannwg UHB. This role focuses on geriatric assessments and care, supporting elderly patients in maintaining independence and improving their quality of life at home.

? Conduct comprehensive geriatric assessments to evaluate mobility, daily living activities, and home safety
? Develop and implement personalized rehabilitation plans to enhance independence
? Provide recommendations for home modifications and assistive devices
? Work closely with physiotherapists, nurses, and social workers to deliver holistic care
? Educate patients and caregivers on self-care strategies and fall prevention
? Maintain accurate documentation of patient progress and treatment outcomes

?? HCPC registered Occupational Therapist
?? Minimum 6 months NHS experience (mandatory)
?? Experience in elderly care, rehabilitation, or community-based therapy
?? Strong assessment and clinical reasoning skills
?? Ability to work independently in a home care setting
