CARE CO-ORDINATOR

Piers Meadows Recruitment are looking for an Occupational Therapists to work in Greater Manchester.

• Band 6 Care Co-ordinator (Occupational Therapist, Social Worker, Nurse)

• To be responsible for the co-ordination, commissioning and personal delivery of individual packages of care for a caseload of service users within CMHTs.

• To undertake initial assessments of new referrals to the service.
