Job Description - Support Worker - Liverpool Shifts

Support Worker
Liverpool
Days, Evenings and Sleep in’s
(7.30am – 3pm, 2.30pm – 10pm, 11pm – 7.30am)
  • Participate in the day to day delivery of a range of services for people with learning disabilities, physical disabilities, autism or associated mental health conditions; working with service users in a person centred way to promote greater choice, skill development, independence and inclusion
  • Work in a variety of community-based settings, including day service activities, supported accommodation and short breaks
  • Contribute to and participate in individual assessment, planning and review processes to ensure that services meet individual needs, choices and personal outcomes
  • Contribute to and participate in risk assessment processes
  • Work with all relevant parties to maintain an environment which is safe and conducive to the overall development and wellbeing of service users
Requirements
  • Level 2 Diploma in Health and Social Care
  • Experience of working with vulnerable people in community or residential based settings
  • Understanding of individual assessment, care planning and review processes in care/ support settings
  • Adult DBS registered to the update service
