Sage Group HealthCare Staffing is seeking Travel LPN LTC for 13-week travel / contract position in Bellaire, MI. Accepting both local and traveler candidates

Position Details:

Position Name: Travel LVN/LPN Long Term Care

Location: Bellaire, Michigan 

Specialty: LTC

Start Date: ASAP

Employment Type: Full-time, Travel, Contract, Temporary

Contract Length: 13 weeks 

Pay Rate: $1400 to $1548 Weekly Gross pay based on a 36 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)



Unit: LTC





Shifts available: Days



Shift timings: 12-hr shifts, Days, 7.00a - 7.30p





Schedule: 36 hours per week guaranteed, 3 X 12-hr shifts per week with Every other weekend, holiday rotation required if scheduled. No block scheduling, No more than 5 days of RTO 

Position Requirements:



  • At least two years of recent/current SNF/LTC or Med Surg: Recent/Current - Required

  • At least 1 travel contract completed - Required

  • Covid Vaccination Proof or exemptions - Required 

Education, License and Certification Requirements:



  • LPN License for State of Michigan - Required

  • BLS Certification from AHA  - Required

Job Description:



  • Administer prescribed medications and treatments in accordance with approved nursing techniques and facility policies and procedures.

  • Prepare equipment and aid the physician during treatments and examinations of residents.

  • Observe residents, record significant conditions and reactions, and notify the supervisor, physician, and family of the resident’s condition and reactions to drugs, treatments, and significant incidents.

  • Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess the condition of residents.

  • Assign duties and coordinate nursing services on the resident unit to ensure resident needs are met in accordance with physician instructions, federal and state regulations, and facility procedures.

  • Consult with the Shift Supervisor on nursing problems and interpretation of facility policies to ensure resident needs are met.

  • Report all complaints or unusual problems to the supervisor and assist with the completion of facility complaint reports.

  • Participate in the orientation, training, and evaluation of nursing personnel.

  • Evaluate nursing activities to ensure resident care, staff relations, and efficiency of service.

  • Observe nursing care and visit residents to ensure that nursing care is carried out as directed and treatment is administered in accordance with physician's instructions.

  • Assume responsibility and maintain records for unit narcotics.

  • Inspect resident rooms and the unit for cleanliness and comfort.

  • Document admissions, discharges, transfers, and incident reports per facility policies and procedures.

  • Participate in the care plan process; complete nursing assessments and resident care plans accurately and timely for assigned residents.

  • Complete special projects and audits at the direction of the Supervisor, CCC, or Director of Nursing.

  • Maintain all educational requirements for the State of Michigan to keep the license current and attend facility in-services and ward conferences as assigned.

  • Maintain resident confidentiality; treat residents with kindness, dignity, and respect; know and comply with all Resident's Rights rules; and promptly report all resident accidents and injuries to the charge nurse.

  • Ensure regular attendance.

  • Perform all other duties as assigned.

  • The facility has a DOG AND a CAT LIVING full-time on site AND BIRDS IN THE AVIARIES.  Applicants must be aware and be comfortable. Thank you.
