Weld is a nonprofit organization committed to providing transitional housing and access to services for individuals with a history of incarceration and those in recovery from drug and alcohol dependency. Additionally, Weld offers employment assistance and resources for the broader re-entry community, clinical case management services and referrals, and re-entry programming and community. Our aim is to ensure the best possible outcomes for this community by addressing the specific barriers they encounter. Our peer-to-peer approach, leveraging lived experiences and expertise, has proven effective in overcoming institutionalized mindsets and addressing broader societal challenges.



Job Summary: 



The Case Manager is responsible for facilitating case coordination services and support through the Weld Clinical Case Management and Housing departments. Case Management services will be provided in-person, as well as via telephone, email, and video call, seeking to diminish barriers to access. The Case Manager will work with new members arriving to Weld via the Mudroom Orientation intake process, as well as those coming into Weld housing. Coinciding with case management services, the Case Manager will support Mudroom programming (orientation and assessments), complete housing visits, and engage in weekly staffing. 


The contract part time employee role is 180 days in duration. In addition to regular progress updates with direct reports, a review of the contract and budgeting for contract renewal will take place 30 days prior to the contract end date. An extension of the contract or transition into FTE status will be contingent on job performance and budgetary approval. 


Good time management is a key component for success in this role. The Case Managers workload will include managing a case load of members across differing departments, and appointments must be regularly scheduled and utilized productively. Accurate documentation is critical for this role. Notes will be used to monitor member progress and to address priorities, challenges, and goals, as well as to inform data, evaluation, and reporting practices. The ideal candidate will be culturally competent, able to recognize and respect the nuances of various cultures and ethnicities. All above-mentioned positions serve at the discretion of the Executive Director and the Board of Directors. 



Duties/Responsibilities


Direct Member Services: 




  • Facilitate and engage in case management services within the Clinical Case Management and Housing departments, seeking to resolve barriers to permanent housing, employment, and other basic needs. 

  • Facilitate and engage in clinical case management services within contract-based housing programs (King County Regional Mental Health & Veterans Court, King County Drug Court, City of Seattle King County Jail Housing Pilot).



  • Manage a caseload of members, supporting their progress towards established goals and plans.



  • Maintain clinical office hours for membership access, engaging in scheduled appointments and outreach processes.

  • Assist in the facilitation of Mudroom programming and assessments, while collaborating with members on the development of measurable and achievable reentry and recovery goal planning.

  • Facilitate trauma informed service termination with members when applicable.

  • Identify and make appropriate, trauma informed referrals to 3rd party providers and organizations in the community for presenting needs. 

  • Complete documentation for member care facilitation and data tracking purposes. 

  • Collaborate closely with team members to foster safe and supportive reintegration environments to help prioritize member needs, encourage peer support, and facilitate the pursuit of individual goals.




Member and Agency Outreach: 




  • Assist with outreach to individuals, institutions, prisons, municipalities, and partner agencies; screening, determining eligibility, enrolling, and orienting of new members and partners. 

  • Engage in care collaboration with outside organizations, agencies, and legal systems for member service delivery.



Organization Operations: 




  • Utilize Salesforce for service delivery, data tracking, and case load management. 

  • Attend weekly team meetings, providing programmatic reports. 

  • Engage in one-to-one meetings with the Clinical Manager or Clinical Director as scheduled.

  • Attend regularly scheduled staff meetings, trainings, and events.

  • Assure that members, staff, volunteers, and community partners are treated with respect and dignity regardless of race, ethnic background, gender, or socioeconomic background.

  • Other duties as assigned.


            Qualifications & Core Competencies: 



  • Graduate-level educational background in Social Work, Mental Health, Counseling, and/or related field.

  • Formal training, certification, or credentialing in Social Work, Mental Health, Counseling, and/or related fields preferred. 

  • A minimum of one year experience providing case management services in a related field.

  • Ability to organize, prioritize, and perform multiple routine and non-routine tasks within a timely manner.  

  • Commitment to diversity, including sensitivity to the needs of residents and staff from diverse cultural and economic backgrounds.

  • Experience in restorative justice, post-prison successful reintegration, substance abuse treatment and recovery preferred. 

  • Demonstrated ability to maintain confidential and sensitive information. 

  • Ability and willingness to work independently and as part of a team; able to make sound judgements without immediate supervision.

  • Strong oral and written communication skills with people and organizations from varying backgrounds and life experience. 

  • Commitment to Weld’s mission and to the transformation of lives through authentic community and supportive services. 

  • Act with honesty and integrity in both your words and actions. 

  • Ability to pass a pre-employment drug screening.
