Job Title: CASE MANAGER (Temporary)
Location: A Woman’s Place Drop-In Center, 211 13th St.
Reports to: Director of Drop-In Services
Status: Non-exempt, Union
Hourly Wage: $30.44/hour
Hours: Monday-Friday, 9am-5pm (Temporary, up to 6 months)

We focus on the hardest to reach, most vulnerable people: Women, Medical Crisis, Substance Use Disorder, and Older Adults. Our programs meet the needs of people, rather than demanding that people meet the needs of our programs.
JOB SUMMARY:
Community Forward SF is hiring a Temporary Case Manager II. The Case Manager II assists clients with needs related to housing, coordination of care to maintain mental and physical health and personal safety, and navigation of the Homelessness Response System. They provide information, education, and referral services within a Drop-In setting. This role is temporary, up to 6 months in duration.
