What You'll Do:

In the Clinical Case Manager role, you’ll be responsible for providing intensive case management services to the most vulnerable at-risk Veterans who have been discharged from the VA Medical Center or other non-VA local area hospitals after a period of acute inpatient care to help achieve a more adaptive level of psychosocial functioning while in the Recuperative Care Program. Working within a comprehensive team of skilled professionals, the case manager and recuperative care team provide intensive support addressing individual Veteran’s needs to help get them back to self-sufficiency.

  • Provides trauma informed intensive case management for severely medically frail and/or mentally ill Veterans, who have been discharged from the VA or non-VA local area hospitals and centers
  • Provides direct service to Veterans in areas including but not limited to self-care skills, adaptive coping, education of the process of illness and recovery, social skills enhancement, healthy lifestyle maintenance, vocational rehabilitation as appropriate and advocacy as deemed appropriate
  • Provides therapeutic, rehabilitative, and recovery services which shall be determined according to the needs of the Veteran as deemed necessary
  • Develops and implements individual care plans addressing the identified individual biopsychosocial needs and services necessary, along with consultation with the Veteran and VA liaison or appropriate VA staff
  • Conducts discharge planning to appropriate long-term transitional or permanent housing, including referrals and application assistance 
  • Responds to crisis situations with Veteran residents and provides access to emergency care as needed 
  • Conducts weekly case conference meetings and, as needed individual or group consultation with the Veteran and RCP staff and VA liaison or appropriate VA staff
  • Maintains a flexible schedule and provides services on-site during hours that best suit the Veteran’s needs and the needs of the VA San Diego Healthcare System as deemed necessary
  • Establishes and maintains collaborative professional relationships with community providers, VASD and other referral sources 
  • Responsible for VA contract activities including outreach, enrollment, placement, training, and follow-up to ensure that all projected goals are met for the contract
  • Displays knowledge of scope of requirements and provides case management services in accordance with the specifications outlined by the VA-VASDHS and Healthcare for Homeless Veterans (HCHV) program contract requirements
  • Monitors all rosters, databases, and logs to ensure compliance within VA contract and guidelines as well as in-house reporting and documentation processes
  • Ensures compliance with program goals/objectives and provides both clinical and administrative required documentation 
  • Transport clients using company vehicle or accompany clients to appointments in the community as deemed necessary 
  • Participates in program staff meetings, trainings and other agency or VA meetings 
  • Performs additional duties as deemed necessary

Requirements

Who You Are:

You're passionate about making a lasting impact, whether supporting individuals directly or enhancing our organizational effectiveness. To thrive in this role, you'll:

  • Master’s Degree in Social Work (MSW) or Master’s Degree with an emphasis in Marriage and Family Therapy (MFT), required 
  • Must be currently registered and in good standing with the State of California Board of Behavioral Sciences as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT) 
  • License-eligible preferred, must obtain license within 6 months of hire
  • 1+ years of relevant work experience, required 
  • Knowledge of substance use disorders, preferred 

THIS IS A TEMPORARY POSITION  (August 4th- January 12th)

Location: Onsite/Escondido

Hourly Rate: $33.00-$38.00

Hours: Monday-Friday: 8:00am- 5:00pm
