Contracted Targeted Case Manager

Morrison Center is seeking a  Contracted Targeted Case Manager/ Care Coordinator at their Brewer Maine Children's Services program. Full Time, flexible to Contracted.

Requirements of the Contracted Targeted Case Manager: 

• Bachelors degree in any field


• 1+ year in Children's Services or 2 + years in social services preferred
  


• Case Management experience is preferred


• Exceptional communication skills


• Strong organizational and writing skills and ability to multitask, prioritize, and problem solve in a fast-paced environment.


• Microsoft Office Suite


• Monday - Friday 8 hours a day in Oakland, Maine
  

Contract Type: Independent Contractor

• Pay: $65/hour


• Contract Duration: Rolling, reviewed annually


• Notice Period: 30 days’ notice required for termination


• Schedule: Flexible hours (Monday–Friday)


• Location: Hybrid with travel for home/community visits in the Brewer area

Responsibilities of the Contracted Targeted Case Manager

• The Case Manager will manage a case load of 15 - 25 clients.


• Helping to manage their appointments


• Education Support


• Transportation needs


• Coordinate and facilitate team meetings.


• Write comprehensive assessments, quarterly reviews and continued stay reviews


• Attend home visits, doctor appointments and family team meetings


• Research, coordinate and communicate with team members
