The Neighborhood Health Advocate is an independent contractor engaged by BBJ/HBCU to connect participants to essential health and social services within the Healthy Beginnings Connect Us / Healthy Start program. This engagement includes direct outreach, participant recruitment, care coordination, developmental screenings, and home visiting in support of pregnant and postpartum women and families. The contractor operates autonomously to achieve the deliverables defined herein, providing trauma-informed, culturally grounded support throughout the program.
Scope of Services & Deliverables
The following describes the deliverable outcomes the contractor is engaged to produce. The contractor determines how, when, and where work is performed to meet these outcomes, subject to any deadlines specified herein.
Participant Support & Care Coordination
Develop individualized service plans in collaboration with the Case Manager, based on each participant's assessed needs, strengths, and goals. Deliverable: completed service plan documentation per participant.
Conduct home visits and virtual support sessions with pregnant and postpartum participants per Healthy Start protocols. Deliverable: visit log and contact notes submitted on a weekly basis.
Provide care coordination and service navigation tailored to each participant's needs and circumstances.
Empower participants by promoting self-sufficiency and advocating for their access to resources and services.
Conduct program and developmental screenings including PHQ-9, GAD-7, and Social Determinants of Health (SDoH) assessments. Deliverable: completed screening documentation submitted per participant per program schedule.
Refer participants to mental health services and other community resources per established protocols. Deliverable: referral documentation submitted within 48 hours of each referral.
Maintain detailed, accurate records of all participant progress and service interactions. Deliverable: records submitted per program documentation schedule.
Outreach & Community Engagement
Conduct ongoing outreach to enroll pregnant and postpartum women within the assigned service area. Deliverable: outreach activity log submitted with each invoice.
Provide health education and resources to participants and the broader community at events and contacts as agreed in the Contractor Services Agreement.
Represent Healthy Start services at tabling and community events as specified in the Contractor Services Agreement. Deliverable: event attendance log and summary submitted with each invoice.
Attend relevant program meetings as necessary to fulfill contracted deliverables.
Contractor Capabilities
Minimum education: High school diploma or equivalent; Associate’s or Bachelor’s degree in a health or human services field preferred
Knowledge, skills, and experience in case management, effective communication, cultural responsiveness, group dynamics, and public health principles
Experience conducting home visits or providing direct services in a community health or social services setting preferred
Familiarity with maternal-child health, postpartum support, and community resources in the Louisville area preferred
Proficiency with Google Suite and client-tracking or case management software
Strong interpersonal and communication skills with demonstrated ability to engage diverse populations
Independent, reliable personal transportation — required
Availability for varied hours including some evenings and weekends as needed to fulfill deliverables
