Job Description - High Priority need - LTC Physical Therapist in Granbury, TX with $2400 Weekly Gross


Sage Group HealthCare Staffing is seeking a Physical Therapist for a travel contract position in Granbury, TX. We are accepting both local and travel candidates.



Position Details:





Position Name: Physical Therapist - LTC

Location: Granbury, Texas



Specialty: PT





Start Date: ASAP

Employment Type: Full-time, Travel, Contract, Temporary

Contract Length: 13 weeks

Pay Rate: Range $2360 to $2400 weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: PT, LTC 

Shifts available: Days



Shift timings:  8 hours, Days, 8.00 am to 4.30 pm





Schedule: 40 hours/week guaranteed, 5 X 8-hours, Monday-Friday, No weekends, No call 

Position Requirements:



  • Physical Therapist experience: 2+ years - Required

  • LTC experience - Preferred 

  • Prior Travel experience – Preferred 

Education, License, and Certification Requirements:



  • PT License for the State applying or Compact if applicable - Required

  • BLS Certification  - Preferred 

Job Description:



  • The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. 

  • They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability and work with the individual to prevent the loss of mobility.

  • Performs patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data.

  • Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders.

  • Conducts ongoing reassessment of the patient and their response to treatment.

  • Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.

  • Supervises and delegates to ancillary staff and students consistent with state laws and department policy.

  • Completes legible, accurate and clear documentation within the established timeframe identified in the department policy and procedure and in accordance with accreditation requirements and insurance regulations.

  • Assesses educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs.

  • Provides an environment conducive to safety for patients, visitors, and staff.  Complies with appropriate and approved safety standards.

  • Functions within scope of practice as defined by the state’s practice act.

  • Performs other related duties as required and directed.
