Sage Group HealthCare Staffing is urgently hiring for a Physical Therapist position in Fort Dodge, IA. Accepting both local and traveler candidates. 

Position Details:

Position Name: Physical Therapist - Outpatient

Contract Length :13 weeks

Pay Rate: Range $2400 to $2600 weekly Gross pay  based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: Outpatient and Acute Inpatient Units 

Shifts available: Days

Schedule: 40 hours/week guaranteed, 5 X 8-hr shifts, Monday-Friday, Weekends - TBD, No call 

Position Requirements:

• Physical Therapist experience in a hospital setting: 1 year+ - Required
  


• Outpatient PT experience – Required 
  


• Inpatient PT experience – Preferred 
  


• Epic EMR 
  


• Covid Vaccination required for this need. Exemptions accepted on case to case basis
  

Education, License and Certification Requirements:

• PT License for State of IA or Compact - Required
  


• BLS Certification from AHA - Required
  

Job Description:

• Provide all aspects of Physical Therapy services in an outpatient setting with 1 day of the week in an inpatient setting.
  


• Performs examination and evaluation to determine diagnosis and prognosis of patient/client and determine appropriate intervention to gain expected outcomes based on patient/client goals including a collaborative approach with patient/family and health care team.
  


• Utilizes of appropriate Physical Therapy interventions to produce desired outcomes.
  


• Utilizes of appropriate documentation and billing to communicate progress and status to physicians, other healthcare professionals and third party reimbursement
  


• Supervision of Physical Therapist Assistant in accordance to state licensure laws.
  


• Utilizes critical thinking in making independent judgements and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
  


• Able to demonstrate the necessary skills and competencies of a physical therapist.
  


• Keeps work area and equipment clean and orderly by maintaining and practicing hospital and departmental policies and procedures regarding safety and infection control
  


• Additional information: Patients per day - 10-12 for Outpatients, 8-10 for inpatient. Team of 4-5 PT and 4-5 PTA. Facility has 12 private rooms and a large gym space. Each visit is scheduled for approximately 45 mins.
