Sage Group HealthCare Staffing is seeking Hospice LPN for a travel/contract position in Blue Ash, OH. Accepting both local and traveler candidates.

Pay Rate: Range $1600 to $1640 Weekly Gross pay based on a 40\-hour work week (Includes Non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Hospice in a home Setting
Shift available: Days
Shift timings: 8\-hour Days, 8.00 am to 5.00 pm
Schedule: 40 hours/week guaranteed. 5 X 8\-hrs, 5 shifts per week, Mon\-Fri. Occassional support for On\-call
Position Requirements:
- LPN experience: 1\-2 years \- Required
- Recent/Current Hospice LPN nursing experience in a home health setting: 1 year within the last 3 years â Required
- Home Care Home Base (HCHB) charting experience â Required
- Must have valid Driverâs License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License, and Certification Requirements:
- LPN License for the State of Ohio or Compact \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide and coordinate care for assigned Hospice patients under general supervision
- Develop and implement nursing care plans for patients as assigned.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure patientsâ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians and IDT team of significant changes in patient status.
- Document and complete charting
