Pay Rate: Range $2560 to $2600 Weekly Gross pay based on a 40 hour work week (Includes Non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Shift timings: 8 Hours; Days, 8.00a \- 5.00p
Schedule: 40 hours/week guaranteed, 5 X 8\-hr shifts per week, Mon\-Fri, 32 productivity points per week expected
Position Requirements:
- Current PT experience: 1\- 2 years \- Required
- PT Home Health experience: 1 year within the last 3 years \- Required
- Home Care Home Base (HCHB) charting experience â Required
- Must be Covid Vaccinated (Declination not accepted \- Exemptions may be considered if signed by Provider/Religious leader)
Education, License and Certification Requirements:
- Bachelor of Science in Physical Therapy or Similar \- Required
- Current State License as Physical Therapist or Compact if applicable \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide skilled physical therapy services to patients in their homes as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to IDT team
- Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
- Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
- Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
- Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
- Communicate with the PTAs and nurse aides on supervisory visit results.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Must meet 30 productivity points per week
- Territories to cover: Mostly Jackson County and Some Clay County
