Sage Group HealthCare Staffing is seeking Hospice RN for 13\-week travel/contract positions in Parsippany, NJ. Accepting both local and traveler candidates.

Pay Rate: Range $2080 to $2200 Weekly Gross pay based on a 40\-hour work week (Includes non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Shifts available: Days
Shift timings: 8 Hours; Days, 10.00 am \- 7.00 pm (Tue\-Fri), 8.00 am â 5.00 pm (Sat)
Schedule: 40 hours/week guaranteed, five shifts per week, Tue\-Sat
Position Requirements:
- Registered Nurse: 2 years \- Required
- Recent/Current Hospice RN experience in a home setting: 1 year within the last 3 years \- Required
- Must be comfortable performing Hospice Admissions
- HomeCare HomeBase (HCHB) charting experience \- Required
- Must have valid Driverâs License and Personal Auto Insurance
Education, License and Certification Requirements:
- ASN or an equivalent â Required
- BSN or an equivalent â Highly preferred
- RN License for State applying or Compact if accepted in the State \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide and coordinate care for assigned hospice patients in the comfort of their homes and primarily perform hospice admissions.
- Develop and implement nursing care plans, including Start of Care and admissions as assigned.
- Complete patient documentation and charting for the assigned caseload before the next business day
- Case management and provide clinical oversight to interdisciplinary
- Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physiciansâ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patientsâ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status
- Document and complete charting for all patient visits within 24 hours.
- Territories to cover: Morris, Bergen and Passaic counties
