Sage Group HealthCare Staffing is seeking Travel Home Health PT for a travel contract position in Pittsfield, MA

Position Details:

Position Name: Travel Physical Therapist \- Home Health

Location: Pittsfield, Massachusetts

Contract Length: 13 weeks 

Pay Rate: Range $3000 Weekly Gross pay based on a 40\-hour work week (Includes Non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: Home Health

Shifts available: Days

Schedule: 40 hours/week guaranteed, 5 x 8\-hour shifts, Monday\-Friday, with rotating weekends, Holiday rotation if required, RTO limit of 5 days

• Current Home Health PT experience: 2 years \- Required
  


• OASIS experience \- Required
  


• Certified Home Health PT experience \- Required 
  


• Acute care experience \- Highly preferred


• Valid DL and Proof of Auto Insurance \- Required 
  


• Prior travel experience: Highly preferred
  

Education, License and Certification Requirements:

• PT License for State of Massachusetts \- Required


• BLS Certification from AHA  \- Required
  


• Home Health Certification – Required (Relias or similar) 
  

Job Description:

•  The physical therapist in this position is responsible for providing physical therapy services to patients in their homes.
  


• The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient’s comprehensive plan of care.
