Key Responsibilities for the PRN Physical Therapist PT:

• 
  

  Provide personalized, home-based physical therapy to patients recovering from illness or injury

  
  


• 
  

  Collaborate with a home healthcare team, which may include RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to create and follow each patient’s individualized plan of care

  
  


• 
  

  Assess patient progress, document improvements, and modify treatment plans as necessary

  
  


• 
  

  Assist patients with exercises and hands-on therapy to manage pain and improve mobility

  
  


• 
  

  Educate patients and their families on the plan of care, therapy goals, exercises, and self-care techniques

  
  


• 
  

  Provide guidance to families to ensure safe and effective assistance with exercises

  
  

Minimum Requirements for the PRN Physical Therapist PT:

• 
  

  Graduate of an accredited Physical Therapist program with an active PT license in PA

  
  


• 
  

  At least one (1) year of physical therapy experience, preferably in home healthcare

  
  


• 
  

  Liability insurance (for independent contractors; excludes full-time employees)

  
  


• 
  

  CPR certification required

  
  


• 
  

  Valid driver’s license and reliable transportation

  
  


• 
  

  Strong clinical judgment, interpersonal skills, and a compassionate approach to patient care
