Job Description - Home Health Speech Therapist

Overview:
As a Home Health Speech Therapist, you will be an integral part of our team, providing high-quality rehabilitative care to patients within the comfort of their own homes.

Job Description:
We are seeking a dedicated and compassionate Speech Therapist to join our team of healthcare professionals. As a Home Health Speech Therapist, you will have the opportunity to make a meaningful difference in the lives of our clients by providing quality speech therapy services in the comfort of their own homes.

Responsibilities:

Conduct comprehensive assessments to evaluate clients’ speech, language, and swallowing abilities.

Develop individualized treatment plans tailored to each client’s needs and goals.

Implement evidence-based therapeutic techniques to improve speech, language, cognition, and swallowing function.

Provide education and training to clients and their caregivers on communication strategies, swallowing techniques, and home exercise programs.

Collaborate with interdisciplinary team members, including physicians, nurses, occupational therapists, and physical therapists, to ensure coordinated care and optimal outcomes.

Document client progress, treatment interventions, and discharge planning in accordance with regulatory standards and company policies.

Maintain professionalism, confidentiality, and ethical standards in all interactions with clients, families, and healthcare professionals.

Requirements:

Master’s degree in Speech-Language Pathology from an accredited program.

State licensure or eligibility for licensure as a Speech-Language Pathologist.

Minimum of 1 year of experience in speech therapy, preferably in a home health or similar setting.

Strong clinical skills and knowledge of speech, language, cognitive, and swallowing disorders across the lifespan.

Excellent communication, interpersonal, and organizational skills.

Ability to work independently and as part of a multidisciplinary team.

Valid driver’s license and reliable transportation for home visits.
