Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans.


• Provide skilled nursing care to patients in their homes, ensuring adherence to best practices and safety standards.


• Monitor patient progress and report changes in condition to the appropriate healthcare professionals.


• Educate patients and their families on disease management, medication administration, and health maintenance.


• Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes.


• Maintain thorough and accurate documentation of all nursing activities and patient interactions.

Requirements

• Valid Registered Nurse (RN) license in the state of Michigan.


• Experience in home healthcare or related nursing fields preferred.


• Ability to work independently and manage multiple patient visits across Wayne, Oakland, and Macomb counties.


• Strong communication and interpersonal skills.


• Reliable transportation to travel to patient homes within the designated counties.
