Homecare RN

University Home Care Inc is seeking a compassionate and skilled Homecare RN to provide high-quality nursing care to patients in their homes throughout Wayne, Oakland, and Macomb counties. This role involves delivering personalized care, conducting assessments, and coordinating with healthcare teams to ensure optimal patient outcomes.

Responsibilities

• Provide skilled nursing care to patients in their homes across Wayne, Oakland, and Macomb counties.


• Perform comprehensive patient assessments to evaluate physical and mental health status.


• Develop and implement individualized care plans based on physician orders and patient needs.


• Administer medications, treatments, and therapies as prescribed.


• Monitor and document patient progress and communicate updates to healthcare providers and family members.


• Educate patients and their families on disease management, medication administration, and preventive care.


• Ensure compliance with agency policies, procedures, and regulatory requirements.


• Collaborate with interdisciplinary teams to coordinate optimal patient care.

Requirements

• Current and valid Registered Nurse (RN) license in the state of Michigan.


• Minimum of 1 year of homecare or nursing experience preferred.


• Ability to travel to homes within Wayne, Oakland, and Macomb counties.


• Strong clinical assessment and critical thinking skills.


• Excellent communication and interpersonal abilities.


• Reliable transportation and valid driver’s license.


• CPR certification.
