Hospice Registered Nurse in Canton, OH

Sage Group HealthCare Staffing is seeking a Registered Nurse Hospice for a 13-week travel/contract position in Canton, OH. Accepting both local and traveler candidates. 

Position Details:

Pay Rate: $1392 to $1440 Weekly Gross pay based on a 24 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Hospice, Home Health

Shifts available: Days

Position Requirements:

• Registered Nurse: 1-2 years - Required
  


• Recent/Current Hospice experience in a Home Health setting: 1 year within the last 3 years – Required
  


• General Inpatient (GIP) experience - Required
  


• Home Care Home Base (HCHB) charting experience – Required 
  


• Must be Covid Vaccinated or provide medical or religious exemption
  

Education, License and Certification Requirements:

• ASN or an equivalent – Required
  


• BSN or an equivalent – Highly preferred
  


• RN License for the state applying or Compact if accepted - Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provides direct nursing care to hospice patients according to established policies, procedures and protocols of the healthcare organization.
  


• Performs accurate and complete admission assessments and reassessments within specified time frame.
  


• Assesses patient for safety risks, abuse and neglect.
  


• Assesses and documents patient’s pain using appropriate pain scale and manages patient’s pain.
  


• Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family and health care team.
  


• Provides supervision, assessment, planning and direct patient care to hospice patients in accordance with agency policies and procedures and the plan of care established by the interdisciplinary team.
  


• Administers medication and treatments safely and accurately with a focus on patient comfort.
  


• Documents changes in patient condition and effectiveness and timeliness of care/interventions.
  


• Evaluates, updates and revises plan of care to facilitate achievement of expected outcomes.
  


• Assesses educational needs of patients, care givers and family considering cultural issues and other special challenges.
  


• Provides teaching and educational material to the patient and/or significant other.
  


• Provides emotional support to the patients and/or significant other.
  


• Provides observation visits as requested.
  


• Other duties as assigned
  


• Territories: Mostly Stark County. Service area is Carrol, Tuscarawas, Wayne, Summit, Stark counties
  


• Orientation will be Mon, Tue, Wed prior the starting the weekend schedule on Saturday
