Long-Term Supports and Services (LTSS) Program Assistant

The Long-Term Supports and Services (LTSS) Program Assistant supports the work of the Medicaid LTSS Home Delivered Meal Benefit and the Healthcare Programs Coordinator. This includes but is not limited to supporting all aspects of home-delivered meals as part of the Medicaid LTSS Benefit. This is a full-time, hourly position; Monday through Friday with core hours of 8:30 am - 4:00 pm; evenings and weekends as needed with advance notice provided.

Essential Duties and Responsibilities
Outreach

• Engage with referral sources
• Conduct presentations and outreach 
• Communicate with eligible clients regarding in-take
• Organize frozen meal & shelf-stable boxes for delivery

Enrollment

• Confirm eligibility
• Process referrals and conduct in-take 

Day-to-Day Operations 

• Communicate with participants about meal delivery(e.g. cancelations, no answer)
• Organize meals & meal boxes for dispatch
• Deliver meals, as needed

Eligibility & Billing

• Check participant eligibility on a monthly basis
• Create LTSS prospect list on a monthly basis 

• Participate in monthly team meeting(s)? Other duties, per the ever-changing needs of the organization 

• 1-3 years of experience in a nonprofit setting
• High School degree or equivalent education or experience
• Experience working with older adults and/or diverse populations 
• Clear results of a background and motor vehicle record check
• Community Health Worker Certification, preferred
