LPN Home Health in Portage, MI with $1500 Weekly Gross

Sage Group HealthCare Staffing is seeking Home Health LPN for a 13-week travel / contract position in Portage, MI. Accepting both local and traveler candidates.

Pay Rate: Range $1400 to $1500 Weekly Gross pay (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home Health

Shifts available: Days

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri.

Position Requirements:

• LPN experience: 2 years - Required
  


• Recent/Current Home Health Nursing Experience: 1 year within the last 3 years- Required
  


• Home Care Home Base (HCHB) charting experience - Required 
  


• Must have valid Driver’s License and Personal Auto Insurance
  


• Must be Covid Vaccinated or provide medical or religious exemption
  

Education, License and Certification Requirements:

• ASN or an equivalent – Required
  


• BSN or an equivalent – Highly preferred
  


• LPN License for State of Michigan - Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.
  


• Assess, Monitor, and chart observations of the patient's condition at each visit
  


• Complete evaluation tasks, including reviewing medication and vital signs under RN's supervision
  


• Provide all aspects of LPN job duties as assigned.
