LPN Home Health with $1720 Weekly Gross in Seattle, WA

Sage Group HealthCare Staffing is seeking Home Health LPN for a travel/contract position in Seattle region, WA. We are accepting both local and traveler candidates.

Pay Rate: Range $1680 to 1720 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Position Requirements:

• LPN experience: 1 - 2 years - Required
  


• Recent/Current Home Health Nursing experience: 1 year required and preferably within the last 3 years 
  


• Home Care Home Base (HCHB) charting - Preferred 
  


• Must have valid Driver’s License and Personal Auto Insurance
  

Education, License, and Certification Requirements:

• LPN License for State of the state applying or Compact if applicable -  Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide skilled nursing care as ordered by the attending physician under RN's supervision.
  


• Observe and monitor patient conditions and notify RN case manager of any changes in the patient’s condition.
  


• Administer medication as prescribed by the physician under RN's supervision and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
  


• Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
  


• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  


• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  


• Attend IDT Team meetings
  


• Territories to cover: North and Northeast of King County
