LPN Hospice with $1600 Weekly Gross in Columbia, MO

Sage Group HealthCare Staffing is seeking a Hospice LPN for a 13-week travel/contract position in Columbia, MO. Accepting both local and traveler candidates.

Pay Rate: $1600 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home Health, Hospice 

Shifts available: Days

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri | On call requirements: Rotate back-up call and take about 4 days a month with at least 1 weekend day/night

Position Requirements:

• LPN experience: 1 - 2  years - Required
  


• Recent/Current LPN Nursing experience: 1 year within the past 3 years - Required
  


• Home Care Home Base (HCHB) charting experience - Required 
  


• Must have valid Driver’s License and Personal Auto Insurance
  


• Must be Covid Vaccinated or provide medical or religious exemption
  

Education, License and Certification Requirements:

• LPN License for State applying or Compact where accepted - Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide and coordinate care for assigned hopsice patients in comfort of their homes.
  


• Implement care plans under supervision.
  


• Empower patients and families to reach healthcare goals by educating them on disease management
  


• Under supervision, provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  Ensure patients’ status is monitored on an ongoing basis and pertinent information is gathered and documented.
  


• Notify RN or Nurse Manager or IDT team member of significant changes in status 
  


• Document and complete charting for all patient visits within 24 hours.
  


• Territories to cover: 60 mins from Columbia office to the East and West.  Approx 20 min to the South and 30 min to the North
