Cornerstone Family Healthcare is actively recruiting for a Medical Transportation Coordinator to join our growing team in Newburgh.

RATE OF PAY/SALARY: $24.26 per hour (35 hours per week) / Bi-weekly pay

WORK LOCATION(S): 40 Saw Mill River Road, UL-5, Hawthorne, NY 10532

STATUS: Full-Time, Temp (Contract ends 2/28/2026)

General Purpose:

Under the supervision of the Program Supervisor, the Medical Transportation Coordinator is responsible for arranging transportation services for clients of the Medical Transportation Program on an as-needed, day-to-day basis. The Medical Transportation Services provides transportation to clients in Westchester, Rockland, and Putnam County.

Key Competencies:

  • Coordinate timely provision of transportation for consumers so that they are able to access HIV-related health and support services.
  • Coordinate and maintain multi-county vendor/provider transportation networks to ensure that they are in compliance with agency standards and requirements.
  • Maintain accuracy of transportation records in order to comply with the Transportation Service Standards and Guidelines of State and Ryan White funded programs.
  • Outreach to other agencies and medical facilities in the multi county area making them aware of our Transportation Program.

 Description of Duties:

  • Conduct outreach to promote  transportation services to PLWHA and to community providers serving HIV-infected individuals especially in underserved communities.
  • Receive requests for transportation service from HVCS staff as well as
  • staff of provider agencies.
  • Screen referred consumers  for eligibility and assess transportation needs to determine what mode of  transportation best meets the consumer’s needs.
  • Process ride requests and  arrange transportation for consumers according to the program’s policies and procedures.
  • Track the status of  transportation services to ensure proper service delivery and appropriate follow-up, as needed.
  • Assist with troubleshooting/following-up on complaints and incident reports; vendor billing reconciliations; monthly report preparation and identify new vendors when needed.
  • Participate in department and agency meetings and regularly scheduled supervision sessions. 
  • Maintain client charts  within program standards.
  • Attend and participate in monthly department, All Staff and other required meetings. 
  • Be familiar with Cornerstone policies and procedures and the Employee  Handbook. 
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations. 
  • Perform other related duties as assigned.

Requirements

  • H.S. diploma
  • 1 year experience working in the field of HIV/AIDS, behavioral  health, substance abuse or other chronic illnesses. 
  • Good organizational and interpersonal communication skills; Working knowledge of PC based computerized database, spreadsheet and word processing applications.
  • Knowledge of local and regional transportation systems or the ability to learn them quickly.
  • Bi-lingual (English/Spanish) an asset.
