Sage Group HealthCare Staffing is seeking Travel LPN LTC for 13-week travel / contract position in Petoskey, MI. Accepting both local and traveler candidates  

Position Details:

Contract Length: 13 weeks 

Pay Rate: $1548 Weekly Gross pay based on a 36 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Shifts available: Nights

Schedule: 36 hours per week guaranteed, 3 X 12-hr shifts per week with Every other weekend, holiday rotation required if scheduled 

Position Requirements:

• At least two years of recent/current SNF/LTC: Recent/Current - Required 
  


• Med Surg LPN experience is accepted if no SNF/LTC experience 
  


• Prior travel experience: Highly preferred
  


• Covid Vaccination Proof or exemptions - Required 
  

Education, License and Certification Requirements:

• LPN License for State of Michigan - Required
  


• BLS Certification from AHA  - Required
  

Job Description:

• Prescribes and adheres to facilities protocols and kindness in all personal and professional interactions while on duty in and around the facility.

•  Under the supervision of RN, LPN assumes responsibility and accountability for a group of residents/patients for a shift of duty.
  


• Nursing care is provided through assessment, implementations, and evaluation of the plan of care.
  


• LPN adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nursing assistants and helpers.
  


• Follows all facilities policies and procedures. 
  


• Performs duties as defined by the State Nurse Practice Act. Follows where applicable JCAHO standards.
