Occupational Therapist

University Home Care Inc is seeking a compassionate and skilled Occupational Therapist to provide exceptional care to patients in Pontiac, MI and the surrounding areas. This role involves working directly with patients to assess, plan, and implement individualized treatment plans that improve their ability to perform daily activities and enhance overall quality of life.

Responsibilities

• Conduct comprehensive evaluations of patients’ physical, psychological, and social needs.


• Develop and implement individualized treatment plans to improve patients' functional abilities.


• Provide therapy services in patients' homes within Pontiac and nearby communities.


• Collaborate with interdisciplinary teams to ensure coordinated and effective care.


• Document patient progress and update treatment plans as necessary.


• Educate patients and their families about therapy goals and techniques.

Requirements

• Licensed Occupational Therapist in the state of Michigan.


• Experience in home health or outpatient therapy preferred.


• Strong interpersonal and communication skills.


• Ability to work independently and manage a flexible schedule.


• Reliable transportation to travel to patient homes.
