Outpatient therapy group that exclusively provides care in the home of the patient. We are based in Naples, FL.

This is not traditional home health, as we are not a home health agency.

Our patients are NOT required to be home-bound.

NO home health agency paperwork/requirements.

Looking for Physical Therapist that would like to have a consistent 2-3 patients, or more, on their caseload.

Do you have your own following of patients? Are you a Medicare Part A Home Health Physical Therapist that would like to be able to continue treating your patients past their discharge date?

There is freedom and flexibility to create your own schedule based

Tailor to your personal and professional needs.

We use an web-based outpatient model EMR

Allows you to be fully mobile

Never are you required to come into the office.
