University Home Care Inc is seeking a dedicated Physical Therapist with Kinnser experience to provide in-home physical therapy services in Westland, MI, and the surrounding areas. This role involves delivering high-quality care to patients in their homes, ensuring personalized treatment plans designed to improve mobility, reduce pain, and promote recovery. Responsibilities

• Conduct comprehensive physical therapy assessments and develop individualized treatment plans.


• Provide in-home physical therapy visits, ensuring patient safety and comfort.


• Document patient progress and update electronic health records using Kinnser software.


• Communicate regularly with patients, families, and healthcare team members regarding treatment plans and progress.


• Ensure compliance with all regulatory, safety, and quality standards.


• Maintain professional development and stay current with physical therapy best practices.

Requirements

• Valid Physical Therapist license in the state of Michigan.


• Experience with Kinnser home care software preferred.


• Previous home care physical therapy experience is highly desirable.


• Excellent communication and interpersonal skills.


• Ability to work independently and manage time efficiently.


• Reliable transportation to travel to patient homes in Westland and surrounding areas.
