Physical Therapist in Chillicothe, OH with $2680 Weekly Gross

Sage Group HealthCare Staffing is seeking PT Home Health for multiple 13-week travel / contract positions in Chillicothe, OH. Accepting both local and traveler candidates.

Position Details:

Pay Rate: Range $2600 to $2680 Weekly Gross pay based on a 40 hour work week  (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health

Shifts available: Days

Position Requirements:

• Current PT experience in an acute care or rehab setting: 1- 2 years - Required
  


• Recent/Current Home Health experience: Highly preferred 
  


• Home Care Home Base (HCHB) charting experience – Preferred
  


• Must be Covid Vaccinated or provide medical or religious exemption
  

Education, License and Certification Requirements:

• Bachelor of Science in Physical Therapy or Similar  - Required
  


• Current State License as Physical Therapist or Compact if applicable - Required 
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.  Observe, record, and report patient reactions, changes, and progress to IDT team
  


• Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
  


• Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
  


• Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  


• Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
  


• Communicate with the PTAs and nurse aides on supervisory visit results.
  


• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  


• Must meet 30 productivity points per week
  


• Territories to cover: Chillicothe, OH and surrounding areas
