Trumen Physicians + Associates is looking for a part-time Post-Acute Care Physician to provide in-person care in skilled nursing and rehab facilities across Bryan and College Station. Your Role

• 
  

  Provide compassionate, in-person care in post-acute settings

  
  


• 
  

  Manage chronic + acute conditions and support care transitions

  
  


• 
  

  Collaborate with facility staff and Trumen’s physician-led team

  
  


• 
  

  Document care accurately in the EMR

  
  


• 
  

  Build strong relationships with patients, families, and staff

  
  

Qualifications

• 
  

  MD/DO with active Texas medical license

  
  


• 
  

  Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics

  
  


• 
  

  1+ year experience in post-acute, SNF, or geriatric care preferred

  
  


• 
  

  Strong communication + independent practice skills

  
  


• 
  

  Reliable transportation within Bryan/College Station
