Sage Group HealthCare Staffing is seeking a PT or a 13 week travel/contract position in Bartlesville, OK. Accepting both local and traveler candidates.

Pay Rate: $2200 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home Health

Shifts available: Days

Position Requirements:

• PT experience: Minimum 1+ years - Required
  


• Home Health PT experience, 12 months within the last 3 years – Required
  


• HCHB Charting experience - Required
  


• OASIS experience - Required
  

Education, License and Certification Requirements:

• PT License for the state applying or compact if applicable  - Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide Physical Therapy services for Home Health Patients
  


• Consult with the physician and an interdisciplinary team in the development of a plan of treatment
  


• Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
  


• Plans and prepares treatment plan based on evaluation of patient data
  


• Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
  


• Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
  


• Drive to patient homes in the assigned territories on a daily basis.
  


• Complete charting and patient documentation in timely manner as per client policies and procedures
  


• Area of coverage: Bartlesville, OK, and surrounding areas.
