Sage Group HealthCare Staffing is seeking a Home Health LPN for a 13\-week travel/contract position in Everett, WA Accepting both local and traveler candidates.

Pay Rate: Range $1680 to $1720 Weekly Gross pay based on a 40\-hour work week (Includes non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health Hospice
Shifts available: Days: 8 Hours;
Shift timings: Days, 8a â 5p
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon\-Fri, Productivity expectation is 30 points per week
Position Requirements:
- LPN experience: 1 \- 2 years \- Required
- Recent/Current LPN Home Health experience: 1 year within the last 3 years â Required
- Home Care Home Base (HCHB) charting \- Required
- Must have valid Driverâs License and Personal Auto Insurance
Education, License and Certification Requirements:
- LPN License for State of the state applying or Compact if acceptable \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide skilled nursing care as ordered by the attending physician under RN's supervision.
- Observe and monitor patient conditions and notify RN case manager of any changes in the patientâs condition.
- Administer medication as prescribed by the physician under RN's supervision and help decrease re\-hospitalizations by educating the patient on preventative measures and making good decisions.
- Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
- Attend IDT Team meetings
- Territories to cover: Snohomish County Ministry (Mostly Everett area and south of that, must be available for 50 mile radius from the office)
